The Future of Vaccine Guidance Hangs in the Balance: Should the AMA Step In?
The world of vaccine recommendations is in turmoil, and the American Medical Association (AMA) finds itself at a crossroads. Should it step into the void left by the dismantling of the CDC’s Advisory Committee on Immunization Practices (ACIP), or focus on restoring the original committee? This question dominated a recent AMA House of Delegates meeting, sparking passionate debate among medical professionals.
Here’s the backdrop: In June 2025, HHS Secretary Robert F. Kennedy Jr. made a controversial move by ousting all 17 members of the ACIP, replacing them with individuals whose vaccine skepticism mirrored his own. This decision sent shockwaves through the medical community, raising concerns about the integrity of future vaccine guidance.
But here’s where it gets controversial... Dave Cundiff, MD, MPH, a delegate for the American Association of Public Health Physicians (AAPHP), proposed a bold solution: the AMA should establish its own vaccine advisory committee, positioning itself as a “trusted, centralized source and public-facing megaphone for science-based vaccine guidance.” While this idea resonated with some, it wasn’t without its critics.
Jason Goldman, MD, president of the American College of Physicians (ACP), argued against the proposal, citing a conflict of interest. The ACP had already authored an emergency resolution urging Kennedy to reverse his decision and restore the original ACIP. Goldman also raised practical concerns, noting the staggering cost of creating a new committee—far exceeding the $3.1 million suggested in the draft resolution. He emphasized that the AMA’s focus should be on unseating Kennedy’s hand-picked committee and reinstating the science-driven ACIP.
And this is the part most people miss... Under the Affordable Care Act, insurance coverage for the CDC’s Vaccines for Children program is tied to ACIP recommendations. Without a legitimate ACIP, the program’s future is uncertain.
Despite differing opinions, both sides share a common goal: restoring reliable, evidence-based vaccine guidance. Cundiff acknowledged the challenge but remained hopeful, while Goldman urged the AMA to prioritize reinstating the original ACIP.
The debate doesn’t end there. Virginia Dato, MD, a pediatric and public health physician, highlighted the need for a coordinated, transparent process to address the rapidly shifting vaccine landscape. With specialty societies, state governors, and other groups issuing their own recommendations, she argued, “we need a unified approach to ensure the best possible guidance for the millions who rely on it.”
Here’s the million-dollar question: Can the AMA fill this void, or is it better to fight for the restoration of the ACIP? Abhishek Dharan, MD, offered a sobering perspective: “I don’t think we’re going back [to the old ACIP]. This nation has fundamentally changed.” He challenged the AMA to rethink how vaccination recommendations are made—should they be subject to political whims, or led by the medical community?
